After years of working in and around the personal injury space, one question keeps coming up in my conversations with medical business operators:
Is your real estate strategy keeping up with how fast PI healthcare is evolving?
For established practices expanding into new markets, growth is no longer just about increasing patient volume.
The bigger challenge is building the physical infrastructure that allows the practice to scale: the right locations, the right facilities, and the right capital strategy, without pulling leadership away from patients and operations.
The Shift to Outpatient Care: Is Your Real Estate Strategy Ready?
As healthcare continues moving toward decentralized outpatient care, the demand for well positioned medical facilities continues to grow.
For PI healthcare operators, I believe this creates three important considerations:
📍 Site Selection
Where should your next location actually be? The right submarket, patient demographics, referral ecosystem, competition, accessibility, and visibility can have a major impact on a location’s long term performance.
🏗️ Facility Delivery
Finding the space is only the beginning. Design, permitting, construction, specialized infrastructure, equipment, and opening timelines can quickly become another business for the operator to manage.
📈 Capital & Scale
Opening one location is very different from building a repeatable strategy for 5, 10, 20, 50, or even 100 locations.
The real question becomes: Can the same expansion model be repeated efficiently from market to market?
That’s where I see the biggest shift happening.
The strongest operators aren’t simply looking for their next location. They’re thinking about how to build an expansion platform that can support many locations.
Real estate should be an infrastructure for growth, not something that slows growth down.
For those operating in the PI healthcare space, I’m curious:
👉 How do you determine where your next location should be?
👉 How much time does your leadership team spend managing real estate and buildout?
👉 Is capital limiting how quickly you can expand?
👉 Do you have a repeatable expansion model that can take you from 5 locations to 10, 20, 50, or even 100 locations?
👉 And most importantly, what is the biggest obstacle standing between where you are today and where you want to grow next?
I’d love to hear what other medical business operators are experiencing.
